These are mandatory forms and if there is a form on this page to meet your needs, you MUST use that form. However, there are a few Dane County-specific forms for Criminal or Traffic Court. If you do not see the form you need listed below, please contact the Clerk of Courts at the location above or the Dane County Legal Resource Center at (608) 266-6316 for assistance.
